1. Please complete this form and return to us prior to start of the course you are applying for. Payment via cash, cheque, Visa or MasterCard will be accepted with the application. Cheques are payable to Kam Lake Academy

2. Completion of “Invoice Sponsor / Employer)” section is considered payment and conditions regarding withdrawal policy apply as if actual payment has been received by Kam Lake Academy

3. Cancellation Notice: Must be received in writing at least 2 calendar days before the start of the course.
